Fatal flaw'ların büyük kısmı üçüncü seviyede çıkar — en sık atlanan seviye de budur. A report can satisfy the code and still be wrong. An independent feasibility review runs on three levels: compliance, technical adequacy and reasonableness. Most fatal flaws surface at the third — which is also the level most often skipped.

Most of what sits on a mining company's financial statements is the accounting translation of a geological decision: units-of-production depreciation divides by reserves, IFRIC 20 stripping capitalisation rests on a defensible block model, and a recovery assumption is only as strong as the geometallurgical test programme behind it. Reconciliation between the resource model and mill feed is the early warning for all of it — the gap appears in the geology data months before it reaches the accounts. A mineral resource estimate is not a cost line; it is the assumption at the top of the balance sheet.
